What is the future of clinic-community-public health electronic referrals and data exchange? Where should we be going and what kinds of new relationships will be forged? What new technology will be needed to effect widespread electronic referrals and data exchange? What new opportunities will unfold then to improve patient care coordination and evaluate health impact?

This webinar will focus on work underway in Massachusetts to create an open-source referral and data exchange program and to pilot clinic-community programs to address these questions. An electronic gateway will also be created for use by community-based programs that have no electronic record system.This work is made possible through a new Prevention and Wellness Trust and a recently awarded CMS State Innovation Grant. The open-source tools developed in Massachusetts will be made available to all states. A description of the process and the timeline for completion will be discussed.

The goal of this webinar is to promote conversation around technologies and relationships emerging in the e-world and already on our doorstep.
